All patients with appointments for health physicals are reminded to bring the required physical form(s) to the appointment and to provide any previous immunization records needed to complete the physical.
Failure to follow these instructions will delay completion of the physical exam !
Be sure to return your completed Immunization record form to us.
Failure to do this will deny you the ability to register for classes.
IF YOU NEED A BLANK COPY OF THE FRESHMAN IMMUNIZATION RECORD AND THE RETURN MAILIING ADDRESS, CLICK HERE »
PA State Law requires you to be immunized against meningitis or sign the waiver form distributed by University Housing.
If you DO NOT comply with this requirement, you WILL NOT be permitted to live in a University facility.
